Through our ACLS contracting process, we build partnerships with organizations that are committed to quality service delivery, compliance, and person-centered care.

From in-home supports to community-based services, our contractors play a critical role in helping individuals remain safe, independent, and empowered in their homes.

Help us gain insight into aging and community needs in Western Wayne County and Downriver through our Multi-Year Plan survey.

The fiscal year (FY) 2027-2029 Multi-Year Plan outlines our priorities and strategies to address the evolving needs of older adults and caregivers in our community. Completed every few years, this plan guides our work over the next three fiscal years by identifying gaps, setting goals, and shaping programs that help older adults remain healthy, independent, and connected.

Your responses directly shape how federal and state funding is prioritized for programs and services that support older adults, adults living with disabilities, and caregivers.

The survey is open to everyone, and community members of all ages are encouraged to participate. Please feel free to share this with your networks so we can hear from as many people as possible.

Falls are the most common injury to people over the age of 60 and the leading cause of hospital admissions. But they can be much more dangerous than that.

Jason and Bryan discuss the services and help that having a Medical Care Alert device can bring to seniors.

They talk about how the device works, what it can be set up to do, and most importantly, they go over all the privacy issues that many people have regarding the Medical Care Alert devices and how to help your senior family member learn that the device can help them with peace of mind.

February Star of the Month: Heather Osmun

Heather is recognized by leadership for her instrumental role in strengthening and streamlining the MI Options program during a period of constant growth and change.

Here’s what MI Options Manager, Kristin Holtz, has to say about her:

“Heather has been instrumental in restructuring Medicare Annual Enrollment for both Volunteers and Options Counselors and is consistently willing to step in and support the MI Options team through any challenge we face. Quite simply, this program would not look the same without her.

The MI Options program often feels like building a plane while flying it at 500 mph. In the midst of this constant change, Heather has been one of the greatest assets to the team. She played a critical role in launching MI Options by leveraging her experience as a SHIP Counselor and her background in long-term supports and services. Heather independently chose to be present at every Annual Enrollment site, ensuring operations ran smoothly, supporting counselors in real time, and taking on reporting responsibilities so others could focus on client care. Volunteers and community members have shared that this was the most organized and streamlined Annual Open Enrollment season they’ve experienced.

Beyond Annual Enrollment, Heather facilitates weekly case consultations, mentors counselors during complex cases, proactively improves scheduling processes, and consistently anticipates the needs of the team. She is graciously receptive to feedback and continually refines her leadership approach. Her expertise, dedication, and steady determination have been a backbone of the MI Options program.”
