Combat Social Isolation at Home With Help from CHATS

The CHATS (Championing Healthy Aging Through Socialization) Program combats social isolation for adults aged 60 and older by providing social support services, friendly visits, friendly reassurance, and linkages to community resources. Our social workers and peer mentors will work with you to set goals that align with your values and meet your needs to combat social isolation where it happens—at home.
