Stay Home Through the MI Choice Waiver Program

The MI Choice Waiver Program provides long-term support and services within the comfort of your own home. Our staff work with you and your family or friends to give you more control over the care you receive.

Services provided through the MI Choice Waiver program include:

  • Community transportation
  • Counseling
  • Home-delivered meals
  • Home modifications
  • Medical equipment
  • Nursing services
  • Personal emergency response system
  • Respite care
  • Supports coordination

See if You or Your Loved One May Qualify

At no cost, the MI Choice Waiver Program gives qualifying aging adults and adults with disabilities a choice over their care. Fill out our referral form to see if you or your loved one may qualify.

Eligibilty

Adults 18 and older must meet the financial and medical eligibility in order to enroll in the program.

Gross income cannot exceed $2,829* per month (excludes spousal income).

Allowable assets limited to $2,000* (excludes one home and one car). Federal Protected Spousal Asset guidelines apply.

We will work with you to determine your medical eligibility.

*Income eligibility changes each year and is 300% of the SSI rate. Contact 734.722.2830 for more information.

Services Provided

Participants must need Supports Coordination (professional support from a social worker or nurse to determine services to help participants stay in their home and community) and one other MI Choice Waiver service. Services include adult day care, community living support, respite care, home modifications, equipment, meals, transportation, and more.
