MI Choice Waiver is a home and community-based program for eligible adults that meet program criteria to receive services and remain in a home or residential setting. Residential settings include an individual’s or family member’s home, assisted living communities, licensed homes for the aged or licensed adult foster care homes.
The program is funded through the Michigan Department of Health and Human Services.

Modeled after the success of our community-based care transitions program, Homecomings provides a patient-centered approach to working with individuals and their families from their hospital admission through their 30-day post-discharge back into their home. We understand that at the heart of patient care is the patient. This is why we work to help empower patients to take a proactive approach to managing their health needs with the goal of avoiding a readmission to the hospital for the same or related reasons they were originally admitted. We strive to provide this enhanced level of service with the utmost quality of care and benefit. By helping patients avoid 30-day readmission, Homecomings not only saves the hospital and the patient money through avoidable medical expenses, it assists with identifying other social determinants in the patient’s home that can be corrected or improved to enhance the patient’s overall quality of life.
